Midline lumbar swelling since birth. Patient has lower limb weakness for about 4 years.

Myelomeningocele and tethered cord

The cord is not terminated at normal level extending inferiority to the L4/5 level where it goes through a bony defect in the posterior arch elements of L5 to S2 vertebra to a large CSF containing sac (tethered cord with myelomeningocele).
No fat content is seen to suggest lipoma.
