Gallstone ileus without pneumobilia

Acute abdominal pain.

Patient Data

Age: 65 years
Gender: Male

Small bowel obstruction secondary to large obstructing gallbladder stone. 

Case Discussion

This is an unusual case of gallstone ileus where there is no pneumobilia or cholecystic gas present. The CT was initially reported as small bowel obstruction secondary to intussusception; however, a 3.3 cm obstructing gallstone (with internal gas) was found at surgery.
