Large right complex cyst with enhancing septations suspicious for malignancy, most likely a cystic renal cell carcinoma. 

Multiple retroperitoneal lymphadenopathy, in the clinical context, suspicious for metastasis.

Histology

The cytomorphologic and immunophenotypic findings are entirely consistent with papillary renal cell carcinoma Type II.
