Acromioclavicular joint injury (Rockwood III)

Presentation

Right shoulder injury sustained while skateboarding, positive piano key sign.

Patient Data

Age: 25 years
Gender: Male
Ultrasound

Initial x-ray upon presentation to the the ER

The right clavicle is displaced superiorly compared to the acromion, the AC joint is widened, the coracoclavicular distance is increased. 

X-ray

Control study a month later

Control x-ray a month later shows anatomical situation following fixation with wire. 

Case Discussion

The findings are in line with a type 3 AC joint injury according to the Rockwood classification system.
