Two year history of convulsions.
Patient Data
Scattered small parenchymal calcifications.
Multiple parenchymal and subarachnoid cysts with small hyperdense "dots".
No significant edema or midline shift.
Cysts have resolved and only thin enhancing wall with hyperdense material remains. Some sites are resolved or barely perceptible.
No significant edema.
Case Discussion
Presentation features active vesicular neurocysticercosis with the "hole with a dot" appearance, where the dot represents the small scolex. The cysts are within the parenchyma and subarachnoid spaces. There are also more chronic findings of calcified nodular disease.
Following therapy, the cysts have resolved and several sites are difficult to detect any residual abnormalities. In the right frontal lobe, the enhancing wall with hyperdense material remains without surrounding edema.
