Acute brucella sacroiliitis

Presentation

Right hip pain and limitation of movement.

Patient Data

Age: 55 years
Gender: Male
mri

Marrow edema at both articulating surfaces of the right sacroiliac joint with postcontrast enhancement. No significant effusion is seen. Normal joint space. No evidence of sclerosis or any chronic changes. There is also associated inflammation of the right iliac and gluteal muscles manifested as abnormal bright T2 signal and post-contrast enhancement.  

Case Discussion

Evident sacroiliitis highly suggested by bone marrow edema and the corresponding postcontrast enhancement. No radiological signs of chronic changes of the sacroiliac joint together with the acute presentation of the patient and the positive lab results for Brucellosis suggest the diagnosis of acute Brucella sacroiliitis
