Acute left parotid sialadenitis

Diagnosis almost certain

Presentation

Tender and swollen left parotid. Background Sjorgens syndrome. ?abscess ?stone in duct.

Patient Data

Age: 55 years
Gender: Female

There is increased density and size of the left parotid and fat stranding adjacent to the left mandible and maxilla. No abscess or stone seen (although artifact from dental fillings significantly affects image quality). Left posterior triangle lymphadenopathy. 

Performed the next day.

ultrasound

Dilated left parotid gland duct and a palpable 3x3mm stone is demonstrated in its most distal portion near the duct orifice. Edematous left parotid gland.

The patient went on to theater and a histologically confirmed sialolith was retrieved.
