Acute uncomplicated appendicitis

Presentation

Presenting with epigastric and right upper quadrant pain for 1 day. Murphy's positive.? Cholecystitis +/- cholangitis? Appendicitis

Patient Data

Age: 35
Gender: Male

The appendix is hyperenhancing and distended measuring up to 10 mm. Surrounding fat stranding. No extraluminal gas, fluid or collection. The remainder of the large and small bowel are normal.

The liver, gallbladder, adrenals, kidneys, pancreas and spleen are normal. The visualized lung bases are clear. No aggressive osseous lesion. 

Case Discussion

Acute uncomplicated appendicitis. The patient proceeded to laparoscopic appendectomy. 

 

HISTOLOGY:

MACROSCOPIC DESCRIPTION: 

"Appendix": An appendix 50x7x5mm with attached mesoappendix 20x10mm.  The serosa is lined by fibrin.  No areas of perforation identified.  Specimen serially sectioned to show yellowish debris within the lumen.  

MICROSCOPIC DESCRIPTION: 

The lumen of the appendix contains acute inflammatory exudate and the mucosa in areas is ulcerated.  There is a transmural acute inflammatory cell infiltrate extending into the surrounding fat.  There is no evidence of malignancy. 

DIAGNOSIS: 

Appendix:  Acute appendicitis.
