Adenomyomatosis of the gallbladder - annular type

Diagnosis almost certain

Presentation

HCC screening, incidental gallbladder finding.

Patient Data

Age: 50 years
Gender: Female

Liver ultrasound

Although showing coarse echotexture, the liver contours are smooth, with no morphological features of cirrhosis. No focal liver lesions. 

Circumferential focal wall thickening at the gallbladder neck is longstanding (similar to previous ultrasound scans back to 8 years ago) and most likely reflects the annular type of adenomyomatosis. A tiny hyperechoic focus could represent cholesterol deposit within. 

Portal vein is patent and has anterograde flow. No recanalization of the paraumbilical vein or signs of varices. 

Normal Spleen.

Although it does not show the typical comet-tail artefact, the features of the gallbladder most likely represent annular adenomyomatosis
