Patient presents with secondary infertility, dysmenorrhea and menorrhagia. The first child was delivered by cesarean birth.

Multiple small diverticula extending to myometrium, especially in the left uterine wall, compatible with adenomyosis.
There are also a cesarean section scar, mild dilatation of the ampullary portion of the left fallopian tube and the contrast media doesn't flow freely around the peritoneal cavity assuming the presence of peritoneal adhesions.
