MRI study shows post-operative changes related to hysterectomy. Multiple small cervical Nabothian cysts are noted
Left adnexal well-defined lobulated multilocular cystic lesion measures about 7x5x6 cm. It elicits a fluid-like signal (low signal in T1 and high signal in T2) with thin internal septae. Thin marginal and septal enhancement is noted in the post-contrast study

Here is a case illustrating the imaging criteria of the O-RADS US 3 and O-RADS MRI 3 categories. Ovarian-Adnexal Reporting and Data System Ultrasound (O-RADS US) and Ovarian-Adnexal Reporting and Data System Magnetic Resonance Imaging (O-RADS MRI) aim to ensure that there are uniform unambiguous sonographic and MRI evaluations of ovarian or other adnexal lesions, accurately assigning each lesion to a risk category of malignancy being present, which informs the appropriate management to be instituted
