Presentation

Chronic migraine.

Patient Data

Age: 30 years
Gender: Female

Small T2/FLAIR hyperintensity in the left temporal periventricular white matter with equivocal clinical significance. 

Incidental short segment fenestration of the distal A1 segment of the right anterior cerebral artery. 

Case Discussion

While intracranial arterial fenestration is on its own a benign developmental anomaly, the altered rheological status and vessel wall irregularity causes an increased risk of developing aneurysms at the affected segment.
