Anterior cerebral artery infarct

Case contributed by Henry Knipe
Diagnosis certain

Presentation

Left sided weakness. History of atrial fibrillation.

Patient Data

Age: 95 years
Gender: Female

On presentation

ct

Loss of grey white matter differentiation involving the right frontal lobe superior sulcus near the vertex is keeping with subacute infarction involving the anterior cerebral artery territory. No territorial infarction is seen elsewhere in the brain. No intracranial hemorrhage.

5 days later

ct

There has been expected evolution of the previously demonstrated right sided anterior cerebral artery territory infarct, with progressive hypodensity, swelling and small petechial regions of increased density consistent with blood product but no large hemorrhagic transformation.

Case Discussion

Shows typical features of subacute infarct in the anterior cerebral artery territory with further evolution. The underlying cause was thought to be cardio-embolic given the history of atrial fibrillation (AF).
