Presentation

Swelling over the anterior fontanelle since birth.

Patient Data

Age: 5 years
Gender: Male
ultrasound

There is a well-defined ovoid mass of hypoechoic heterogeneous echotexture with posterior acoustic enhancement.

The mass is of subgaleal location at the midline over the anterior fontanelle of intermediate signal on T1WI, and high signal on T2WI/FLAIR with no enhancement seen. No communication with the underlying intracranial structures. The adjacent segment of the superior sagittal sinus is patent.

Case Discussion

The ultrasound and MRI features are most consistent with an anterior fontanelle inclusion cyst, also called anterior fontanelle dermoid/epidermoid.
