Case contributed by Hidayatullah Hamidi
Diagnosis almost certain

Presentation

Patient Data

Age: 5 years
Gender: Male
ct

Elongated aortic arch located high in the mediastinum. The part of the arch distal to the origin of the left subclavian artery is slightly dilated.

A small patent ductus arteriosus (PDA)  between the aortic arch and pulmonary trunk measuring approximately 2mm in the maximum transverse diameter.

The right subclavian artery is arising from the aortic arch distal to the origin of the left subclavian artery and courses posterior to the trachea to the right side.
No evidence of chest wall collateral formation.
Normal pulmonary artery and its branches.

Case Discussion

The presence of aortic elongation, narrowing, or kinking with no pressure gradient or collateral formation is suggestive of aortic pseudo-coarctation than true coarctation.
