Arachnoid cyst - middle cranial fossa

Presentation

Right hemiparesis

Patient Data

Age: 15 years
Gender: Female

A large extra-axial cystic structure of CSF signal is seen on all sequences, filling the whole left middle cranial fossa, extending superiorly along the Sylvian fissure. Note how pulsation fo the internal carotid results in turbulent flow in the cyst with signal loss on T2 (best seen on coronal images). Mass effect is noted on the frontal and temporal lobes as well as the midline structures. Remodeling and thinning of the adjacent bone are noted. No restricted diffusion is seen on DWI/ADC.
