Arachnoid cyst - middle cranial fossa (Galassi type II)

Presentation

History of a bullet injury in the right lower leg. The patient developed unconsciousness.

Patient Data

Age: 30 years old
Gender: Male

Well defined cystic space returning CSF signal intensities on all sequences. The cystic space has slightly displaced the left temporal lobe posteriorly. There is superior extension of the cystic space along the Sylvian fissure.

No bulging of the left temporal bone.

Case Discussion

The cystic space is returning CSF signal intensities and extending to the Sylvian fissure. The features are suggestive of an arachnoid cyst, Galassi type II. 

DWI and ADC maps may be required to rule out the epidermoid cyst, which would show diffusion restriction.
