Atypical vertebral pedicle hemangioma

Patient Data

Age: 45 years
Gender: Female

There is evidence of abnormal T2/STIR high signal and T1 low signal lesion of the left pedicle of the L4 vertebra. The lesion has persistent high signal on fat saturated images (STIR - fat sat) No significant expansion is noted. No extension to the soft tissues is seen.

The lesion visible in the left pedicle of L4 (16 xc 15 x 13 mm) showing thickened-trabeculae pattern in CT images. No significant cortical thickening or bony destruction is seen. No extension to the soft tissues is noted.  No suspicious periosteal reaction is seen.

Case Discussion

Typical vertebral hemangiomas show T1 and T2 high signal with loss of signal on fat saturated sequence -due to fat rich nature-, while this lesion does not. However the CT features are more typical for hemangioma.
