Linear horizontal lucency at the base of the 5th metatarsal. 

Diagnosis: Fracture at base of 5th metatarsal.  

Case Discussion

Linear lucency at the base of the 5th metatarsal can be confusing especially in the pediatric age population. One might mistake this as normal apophysis. To differentiate apophysis from fracture, the fracture is oriented perpendicular to the shaft of the 5th metatarsal (horizontally). The normal apophysis is oriented vertically or parallel with the metatarsal.
