Benign non-simple ovarian cyst

Presentation

History of lung cancer. Adnexal mass seen on staging CT. No relevant symptoms.

Patient Data

Gender: Female
ultrasound

Unilocular lesion in the left ovary measuring 2.2 cm in maximal diameter. It contains several small echogenic solid components, all measuring less than 3 mm.  As such, they do not meet criteria of being labeled as papillary projections, based on IOTA definitions1.

Case Discussion

Unilocular left ovarian lesion containing small echogenic small mural solid components up to 3mm.

Based on IOTA simple rules, this is a benign feature (B2 - solid components less than 7mm). There are no malignant features.

ADNEX model: 94% chance benign

O-RADS: category 2 (almost certainly benign) - recommend follow up in 1 year.

Conservatively managed; lesion was stable at 12 months follow up.
