Schizencephaly - bilateral open lip

Diagnosis probable

Presentation

Quadriparesis recently, some mental retardation. No previous imaging.

Patient Data

Age: 20 years
Gender: Male

Grey-matter lined CSF clefts extending from pial surface to ependymal surface, with associated polymicrogyric smooth surface along clefts. Both are open-lip. Partially deficient septum pellucidum was also suggested.

Case Discussion

This case depicts bilateral schizencephaly. An important differential is a porencephalic cyst, which is classically described as encephalomalacic cysts communicating with the ventricle. In this case, a hypointense signal cleft lining is seen along the ependymal surface, which is usually absent in porencephalic cyst.
