Bilateral schizencephaly (open lip and closed lip)

Quadriparesis recently, some mental retardation. No previous imaging

Patient Data

Age: 20 years
Gender: Male

Grey-matter lined CSF clefts extending from pial surface to ependymal surface, with associated polymicrogyric smooth surface along clefts. Right sided cleft is open-lip, while left side cleft appears apposed (closed lip). Partially deficient septum pellucidum was also suggested.

Case Discussion

This case depicts bilateral schizencephaly.

Important differential is porencephalic cyst, which are classically described as encephalomalacic cysts communicating with ventricle. In this case, a hypointense signal cleft lining is seen along ependymal surface, which are usually absent in porencephalic cyst.
