The brainstem is diffusely enlarged by a mass with low signal intensity on T1WI, heterogeneously high signal intensity on T2WI/FLAIR and a central area of irregular peripheral contrast enhancement. Mass effect is demonstrated with effacement of the 4th ventricle and obliteration of the prepontine and ambient cisterns.
Case Discussion
The features are most consistent of high grade brainstem glioma.
