The MRI sequences demonstrate engorgement of the right cavernous sinus, dilated ipsilateral superior ophthalmic vein, and an abnormally dilated flow voids vascular structures in the anterior middle cranial fossa, indicating a caroticocavernous fistula (CCF).

On MRA the presence of high flow communication between the intracavernous portion of the right ICA and the cavernous sinus is seen, indicating most likely a direct CCF.

Case Discussion

MRI features of a caroticocavernous fistula (CCF).

Caroticocavernous fistulas (CCF) represent an abnormal communication between the carotid circulation and the cavernous sinus. They can be classified into two types direct and indirect according to their various features.
