Central fracture-dislocation of the acetabulum

Diagnosis almost certain

Presentation

Fell from height onto the right leg. Severe pain, shorter lower limb and reduced movements of the hip.

Patient Data

Age: 75 years
Gender: Male

X-ray showing a fracture of the right acetabulum with central dislocation of the femoral head.

Right ilioischial and iliopectineal lines are completely disrupted.

Case Discussion

Displaced central fracture-dislocations of the hip are serious injuries. Prone to result in permanent disability. They are caused by high kinetic energy.

The classification system of Judet and Letournel has led to improved management of such injuries.

Radiological assessment should include: the anteroposterior view, the iliac oblique view and the obturator oblique view. CT gives much additional information regarding eventual impacted fractures of the acetabular wall, retained bone fragments in the joint, and the degree of comminution.
