Presentation

45 days of headache accompanied by altered mental status and fever.

Patient Data

Age: 25-30 years
Gender: Female

MR images show hyperintense lesions in the left basal ganglia. Numerous hyperintense lesions also are seen around the gray matter-white matter junctions, findings indicative of a hematogenously disseminated process.

Diffusion-weighted MR image shows high-signal-intensity rims and low-signal-intensity centers of the lesions.

Unenhanced T1-weighted MR image shows hyperintense borders, a somewhat irregular and “shaggy” appearance, and low-signal-intensity centers of the lesions

Case Discussion

Our patient was empirically treated for toxoplasmosis and had a good clinical outcome.
