Chiasmatic-hypothalamic cavernoma

Presentation

Visual disturbances.

Patient Data

Age: 20 years
Gender: Female

Small well-defined, lobulated suprasellar mass centered on the optic chiasma and hypothalamic region, extending to the interpeduncular cistern and along the optic tracts. It displays a mixed signal intensity centrally on both T1 and T2 with a low signal rim and blooming on T2* sequence as well as small bleeding along the left optic tract.

on T1, high signal T2 with vivid homogeneous enhancement on postcontrast sequences. A mass effect is noted on the midbrain and temporal lobes. No significant obstructive hydrocephalus is seen. Mild heterogeneous enhancement is noted on postcontrast sequences.

Normal appearance of the pituitary gland with a mass effect on the pituitary stalk.

pathology

Conclusion of the pathological report compatible with cavernoma.

Case Discussion

MRI features of a small suprasellar soft tissue mass of mixed signal intensity on both T1 and T2, centered on the optic chiasma and hypothalamic region.

 The patient went on to have surgical resection of the tumor with a histopathological exam that confirm the diagnosis of cavernoma (type II in this case according to Zabramski classification).
