Abdominal pain in right hypochondrium. Previous history of cholecystectomy.

Filling defects are seen within the biliary tree on thin cross-sectional T2 weighted image.

Case Discussion

MRI T2 weighted imaging: filling defects within the dilated common bile duct. Surgical absence of the gallbladder.

MRCP (MRI cholangiography) has largely replaced ERCP as the gold standard for diagnosis of choledocholithiasis, able to achieve similar sensitivity and specificity (approaching 100%) without ionizing radiation, intravenous contrast or complication rate inherent in ERCPs.
