Choroid plexus xanthogranuloma

Diagnosis almost certain

Presentation

Headache

Patient Data

Age: 75 years old
Gender: Male

Few lesions are noted in the choroid plexus regions are noted bilaterally. The lesions returning low signal in T1 and moderately high in T2/ FLAIR sequences.  Diffusion restriction is noted in DWI and corresponding ADC map images.
Multifocal T2/FLAIR high signal intensities are noted in periventricular white matter with no diffusion restriction.
Significant brain parenchymal involution is noted which is not unusual in this age.
No hydrocephalus.
The interhemispheric fissure is centered on the midline.
No abnormalities are seen in the basal ganglia, internal capsule, corpus callosum, or thalamus.
The brain stem and cerebellum show no abnormal changes in signal characteristics.
No abnormality is noted in the sella, pituitary gland or suprasellar structures
The cerebellopontine angle areas appear normal on each side. The orbital contents are normal.
No abnormal enhancement was detected after an IV injection of contrast.

Case Discussion

The features are suggestive of benign findings and can be seen in entities like choroid plexus xanthogranuloma. The differential is adult choroid plexus cysts.
