Chronic submandibular sialadenitis

Case contributed by Varun Babu
Diagnosis almost certain

Presentation

Recurrent bouts of bilateral submandibular swelling

Patient Data

Age: 40 years
Gender: Female
ultrasound

Diffusely swollen submandibular glands replaced by numerous parenchymal cysts, typically seen in chronic salivary gland inflammation.

Symmetrically bulky bilateral submandibular glands with maximal cross section dimensions of 31 x 24 mm and 27 x 21 mm on right and left sides respectively. Couple of subcentimeter bilateral level Ib nodes. Dynamic post i.v. gadolinium contrast shows symmetric progressive persistent enhancement of both glands.

Case Discussion

The lack of a focal lesion, with diffuse cystic change of both glands with relatively preserved internal architecture favors diagnosis of chronic bilateral sialadenitis
