Presentation

Abdominal pain.

Patient Data

Age: 50 years.
Gender: Male

Multiple air-fluid levels ascending colon with dilated transverse colon. Minimal gas descending colon.

Enlarged ill-defined pancreas with peripancreatic fluid and other scattered abdominal fluid collections. Soft tissue stranding in the central mesenteric fat. Thickened second and third portions of the duodenum. Dilated transverse colon with abrupt caliber change at the splenic flexure with decompressed descending colon.

Case Discussion

A dilated colon with abrupt caliber change at the splenic flexure is called the colon cut-off sign and may occur in patients with abnormalities in the left upper abdomen, in this case acute pancreatitis.
