Complicated lung hydatid cyst

Diagnosis almost certain

Presentation

Fever, chest pain and productive cough for 14 days, Right side decreased breathing sounds.

Patient Data

Age: 15 year
Gender: Male
x-ray

Relatively thick-walled air filled cavity in the right hilar and infrahilar region involving medial aspects of right middle and lower zones. 

There is a large air filled cavitary lesion with internal crumpled membrane in right lung lower lobe. Surrounding mild pneumonitic changes are seen.
Right side mild pleural effusion is seen.
Rest of the Lung fields are clear.
Prominent right hilar and sub cranial lymph nodes are seen. The largest right hilar lymph nodes measures 16 mm in short diameter.

Case Discussion

Living in an endemic area of echinococcus granulosus and the CT features of crumpled internal membrane, the features are very likely for complicated hydatid cyst with bronchial communication.
