Cortical desmoid

Case contributed by Craig Hacking
Diagnosis almost certain

Presentation

Sports injury.

Patient Data

Age: 20 years
Gender: Male
x-ray

Alignment is normal. No fracture or joint effusion. A well-defined lucency at the posterior cortical margin of the medial distal posterior femur is in keeping with a cortical desmoid.

Case Discussion

Cortical desmoid is a do not touch lesion. It represents a 'tug' lesion, due to repetitive stress at the attachment of the medial head of gastrocnemius or distal adductor magnus at the posterior medial aspect of the distal femoral metaphysis
