Presentation

Knee pain post motor bicycle accident.

Patient Data

Age: 20 years
Gender: Male
x-ray

Normal alignment. No fracture or joint effusion.

ct

Knee joint alignment is normal. No joint effusion or lipohemarthrosis. Subcutaneous soft tissue edema and stranding in the anterior medial aspect of the knee at and below the level of the patella.
No fracture identified.

Tiny incidental bone island in the anterior aspect of the medial femoral condyle.

Incidental irregular 10 mm round cortically based lesion in the posterior medial aspect of the distal femoral metadiaphysis at the attachment site of the medial head of gastrocnemius is consistent with a cortical desmoid. No associated periosteal reaction.

IMPRESSION

  • No fracture or malalignment.
  • Incidental cortical desmoid (Bufkin lesion) which does not require any further follow-up or treatment.

Case Discussion

Cortical desmoids are 'do not touch' lesions as under microscopy they can mimic malignancy. This case is an excellent CT example although the lesion is not evident on the radiographs.
