Crossed fused renal ectopia

Case contributed by Aditya Shetty
Diagnosis almost certain

Presentation

Abdominal discomfort. On US, the absence of left kidney in the left renal fossa was noted and was actually located in the pelvis on the right side.

Patient Data

Age: Adult
Gender: Male
Fluoroscopy

Intravenous urography (IVU) shows absence of left renal shadow in the left paraspinal region on the plain x-ray. Post contrast administration, the kidney is seen to have crossed the midline and lies on the right.

Serial images shows that both kidneys have their respective pelvi-collecting systems, with normal openings of both ureters into the urinary bladder.

Case Discussion

Often described as the kidney having crossed the midline. However, it is an anomaly wherein the kidney has not ascended during embryogenesis.
