Cystic degeneration of the peroneus brevis tendon

Presentation

Recalcitrant pain in the lateral heel. No history of trauma.

Patient Data

Age: 65
Gender: Female

Th peroneus brevis tendon shows a longitudinal split tear at the malleolar level with anterior migration of the longus tendon. The course of the brevis tendon proximal and distal to the lateral malleolus shows a tubular transformation with a thin outer hypointense shell containing fluid equivalent signal.

Case Discussion

The findings are probably caused by extensive mucoid degeneration of the tendon fibers in chronic attrition damage. They are identical to intratendinous ganglia described in a few case reports in the literature1.
