Dentigerous cyst

Presentation

Left sided headaches for 2 months. No jaw swelling or pain. no previous sinus surgery or dental extraction.

Age: 37
Gender: Female

There is an uninterrupted right third molar tooth with the roots projecting into the maxillary antrum. There is no associated odontogenic sinusitis identified.There is an ectopic /unerupted third molar tooth seen projected within the left maxillary antrum with associated surrounding soft tissue density (50HU) interspersed by ill-defined calcific density. This presumed dentigerous cyst is unilocular and measures approximately 17 x 25 x 12 mm. The bony inferior margin of the cyst appears to communicate with the presumed 38 root cavity. The margins of the cyst is thick walled.

The osteomeatal complex is patent bilaterally. 

Case Discussion

There are appearances in keeping with a left dentigerous cyst extending into and involving most of the left maxillary antrum. The differential of a odontogenic keratocystic odontic tumor (KCOT) is included.
