Presentation

Diabetic foot.

Patient Data

Age: 60 years
Gender: Male
mri

2nd to 5th toes are not seen (amputation).

Bone marrow edema (low signal on T1 and high signal on T2/PDfs sequences), bony defect and cortical disruption at second to fifth metatarsal bones are seen.

Bone marrow edema(high signal T2/PD fs sequences) at first metatarsal bone and first toe proximal phalanx are seen.

Severe soft tissue edema at forefoot and midfoot is noted.

Signal void foci at forefoot soft tissue in favor of anaerobic cellulitis.

Case Discussion

Regarding the imaging findings, first metatarsal and great toe osteitis, as well as 2nd to 5th metatarsal osteomyelitis, are the main diagnoses.
