Moderate four chamber enlargement is seen with thinning of ventricular
myocardium, and annular dilatation of mitral and tricuspid valves. IVC
and pulmonary veins are prominent.

In a young alcoholic, dilated cardiomyopathy is one of the most important causes of four chamber cardiac enlargement.
