Presentation

Intermittent back pain with no previous trauma or surgery.

Patient Data

Age: 40 years
Gender: Male

Dorsal indentation and anterior displacement of the mid-thoracic spinal cord giving scalpel sign associated with focal Increased cord signal on T2WI.

Case Discussion

The thoracic spinal cord is displaced anteriorly with subtle myelopathy on T2WI.

Differential diagnosis included ventral cord herniation (spontaneous or with prior surgery) and spinal arachnoid cyst.
