Double J stent perforating the pelviureteric junction

Presentation

Known for obstructive uropathy. Had ureteroscopy 15 days ago with extraction of a stone from the right renal pelvis and insertion of a double J stent. Now the patient presented with persistent right flank pain.

Patient Data

Age: 35 years
Gender: Male

The CT urography demonstrates:

  • right hydronephrosis with calyceal rupture and extravasation of contrast medium in the renal parenchyma
  • perinephric fat stranding with thickening of the adjacent peritoneal reflections
  • the upper end of the double J stent is lying outside the renal cavities through a perforation of the pelviureteric junction with mild extraluminal periureteric contrast at the level of the right transverse process of L3. 

Normal appearance of the left kidney and urinary bladder.

Case Discussion

The most common complications of ureteric stents include 1,2:

  • urinary tract infections
  • migration
  • failure to insert/blockage
  • encrustation
  • stent fracture
  • renal parenchymal perforations are rarely reported in the literature
