Presentation
Patient (G2P1) presented with vaginal bleeding during first trimester pregnancy. Gestational age by LMD is 8 weeks.
Patient Data
A gestational sac seen within the endometrial cavity showing an embryo with no cardiac pulsations (CRL = 6.1 mm which corresponds to 6 week + 3 days gestation). An echogenic yolk sac (4.5 mm in diameter) and echogenic chorionic cavity were also noted.
Case Discussion
An echogenic yolk sac is an indeterminate abnormality that should not be considered alone for the diagnosis of failed early pregnancy
In this case, echogenic yolk sac and chorionic cavity along with embryo CRL <7 mm with absent cardiac pulsations are suspicious of failed early pregnancy
Follow up ultrasound was done after 7 days and revealed absent fetal cardiac pulsations and diagnosis of failed early pregnancy was confirmed.
