Extension of CSF signal intensity(T1 hypointense, T2 hyperintense) noted into sella with displaced rim of pituitary tissue inferiorly. Infundibulum is extending through the CSF signal upto floor of sella, suggestive of empty sella which is detected incidentally.

Case Discussion

Empty sella is extension of subarachnoid space into sella turcica, with or without enlargement of sella. Infundibulum sign is positive in empty sella which is extension of infundibulum upto floor of sella.

Differential diagnosis include - cystic tumor and herniated 3rd ventricle.

Secondary empty sella may be seen post surgically.
