There is expansion of right greater wing of sphenoid bone which appears hypointense in all sequences, likely hyperostosis, and shows dural thickening and enhancement surrounding it, extend to the cavernous sinus.  It causes regional mass effect, protrudes into the right orbital cavity, splaying adjacent extraocular muscles, and compressing the right optic nerve intracanalicular part.

Case Discussion

Bony expansion and hyperostosis with surrounding dural enhancement extend to the cavernous sinus, and bulged on the lateral orbital wall, subsequently causing right proptosis and right optic nerve compression, suggestive of sphenoid meningioma en plaque.
