Moth eaten, permeative lytic lesion in the right petrous temporal bone involving the vestibular aqueduct.

T1 hyperintense, T2 hyperintense, multilobulated mass in the right petrous bone at the location of the vestibular aqueduct. DWI not shown and normal. T2* imaging is noncontributory due to artifact from air in the adjacent mastoid air cells.

Case Discussion

Moth eaten or permeative lytic appearance in the region of the vestibular aqueduct is diagnostic of endolymphatic sac tumor. This case has a small tumor that is intrinsically T1 and T2 hyperintense. The patient did not have VHL. This was a sporadic case.
