Epididymal appendix

Presentation

Chronic intermittent left testicular discomfort. His only past medical history was of a left inguinal hernia.

Age: 35 years

There is a short pedunculated mass arising from the head of the left epididymis.

Case Discussion

Although no cause was seen for the patient's testicular pain, an incidental finding of a left epididymal appendix was seen.

The appendix testis and appendix epididymis are vestigial remnants of the Mullerian and Wolffian ducts, respectively. They are common incidental findings with the appendix testis reported to be present in 90% of men and the appendix epididymis in 25% of men. Both can undergo torsion, resulting in acute scrotal pain.
