Axial CT images demonstrate a left juxtacardiac encapsulated fatty lesion with soft tissue rim and intrinsic and surrounding soft tissue stranding. 

Note the focal thickening of the adjacent pericardium.

Coronal reformatting image shows the encapsulated fatty lesion with intrinsic and surrounding tissue strands adjacent to the left epicardiac fat pad.

Case Discussion

Findings are compatible with epipericardial fat necrosis.

The other considerations include: pericardial/epicardial lipoma or liposarcoma.
