Epiploic appendagitis

Presentation

Left iliac fossa pain, tenderness, guarding.

Patient Data

Age: 35 years
Gender: Male

At the junction of the descending colon and sigmoid colon there is a small ovoid fat-density structure with a thin enhancing rim and surrounding inflammatory stranding. This appearance is consistent with epiploic appendagitis.

No diverticula are seen in the colon and given the young age of the patient, diverticulitis is unlikely.

Liver, spleen, kidneys, adrenal glands and pancreas are normal in appearance. No free fluid or free gas. Surgical clips in the inguinal regions bilaterally suggest previous bilateral hernia repair.
