Presentation

Lump in the right breast.

Patient Data

Age: 39 years

Breast US

Circumscribed ovoid solid nodule that is hypoechogenic and has some small echogenic foci.

Magnified CC view of the right breast demonstrates nodular cluster of amorphous microcalcifications in the superior inner quadrant, consistent with the lesions identified on ultrasound. 

Breast US to guide core-biopsy

Core-biopsy was performed using 14G needle. The nodule has remained stable in appearances since the last scan.

Mammography confirms the metallic clip location within the targeted nodule. The core fragments were also radiographed showing inclusion of the microcalcifications. 

MACROSCOPIC DESCRIPTION: "Right breast core biopsy 12 o'clock 1cm from nipple": Four white and yellow fibrofatty breast core biopsies 9mm, 10mm, 12mm and 13mm. A1.

MICROSCOPIC DESCRIPTION: Sections show fibrofatty breast core biopsies. The cores show a densely collagenous lesion with a distinct border. Set within the collagen are scattered epithelial spaces, some of which demonstrate usual ductal hyperplasia and apocrine metaplasia. There is no evidence of in situ of invasive malignancy.

DIAGNOSIS: Right breast nodule, 12 o'clock, 1cm: Fibroadenoma.

Case Discussion

This case illustrates a histologically proven fibroadenoma with microcalcifications, this appearance on imaging makes differentiation from malignancy difficult.
