History of previous sinonasal operation, still complaining of headache and nasal discharge.

Right frontal osteoma, left antrochoanal polyp and bilateral middle turbinectomy. Mucoperiosteal thickening of para nasal sinuses sparing the sphenoids which compatiblewith chronic sinusitis.
