Giant lateral thoracic meningocele

Presentation

Paraplegia.

A large intraspinal extramedullary cyst with CSF signal extending from T10 to L2.

A giant meningocele is noted extending in the right paraspinal region of the right hemithorax measuring 23 x 17 x 16 cm in dimensions. Smaller left paraspinal meningocele is noted at T10 level connected to the intraspinal cyst.

Coronal myelogram shows a giant right meningocele and multiple small paraspinal meningoceles.

Case Discussion

The patient was referred for evaluation of paraplegia secondary to multiple intra and extraspinal cysts. They underwent lumboperitoneal shunt.

This case has posed a challenge to the surgeons.
